
The National Safety and Quality Primary and Community Healthcare Standards (Primary and Community Healthcare Standards) are applicable to all healthcare services that are directly involved in patient care in primary and/or community setting.
The purpose of this form is for Quality Innovation Performance Limited (QIP) to understand the scope of the services provided by your primary and/or community healthcare service(s). This understanding will allow QIP to quote for accreditation services against the the Primary and Community Healthcare Standards.

There are three models of assessment available for the Primary and Community Healthcare Standards. This flexible approach recognises the varying stage of readiness and engagement across the sector. In most instances, a healthcare service may choose what model of assessment to undertake. Please note: as per the Scheme requirements outlined by the Australian Commission of Safety and Quality in Health Care (the Commission) due to the particular risk some healthcare services might not be eligible for the selected assessment.
